To save parents time, I only need 1 form per family completing, (rather than one for every child, for every course). So if booking multiple children/or multiple courses with the same details, complete just 1 form with all the information on or just text me the childrens' names and dates wishing to attend, and we can complete the form on the first date of attendance.
